FOR CARE CODE OF CONDUCT

For Life Pty Ltd (Australian Company Number (ACN) 149 645 885) producer of the For Care Online Platform product (“For Care”) (“For Care”, “we”, “our” or “us”) is committed to providing a safe and respectful Online Community for all involved. Any reference to “For Care” within this document is a reference to both For Life Pty Ltd and the For Care Online Platform, inclusive.

It is of the utmost importance that all those that access the For Care Online Community respect and uphold the rights of persons ageing or with disabilities to have independence, safety and quality of life. Also, that all within the Online Community uphold the rights of every worker to a safe and professional environment. This means promoting dignity, choice, control, respect, professionalism, health and safety, open communication, ethical behaviour and freedom for all. 

  1. Definitions

    1. Account Delegate means Users who are authorised to manage the account of Customers or Providers on their behalf.

    2. Admin Personnel means Users who are employed by For Care to undertake account monitoring and verification tasks on our Site.

    3. Customers means Users who register to obtain services or supports available on our Site.

    4. For Care Online Community or Online Community means all Users of our Site; being, Admin Personnel, Account Delegates, Customers, Providers and Visitors.

    5. Providers means Users who register on our Site to deliver services or supports to Customers.

    6. Site means the For Care website located at www.forcare.net.au.

    7. Users means any person who accesses, registers and or logs onto our Site.

    8. Visitors means Users who access our Site but do not create an account as a Customer or Provider.

ARE YOU A FOR CARE CUSTOMER?

As a member of our For Care Online Community you have registered to connect with supports and services offered by For Care Providers. Whilst we appreciate you are choosing a service that promotes control, choice and self-direction or self-management, this also means you will be expected to be responsible for your choices. As such, this means you agree to:

  1. Communication

    1. Ensuring all written and or verbal communication, whether via our Site or in the physical presence of others, is respectful, open and honest (whilst remaining in line with confidentiality and privacy. We expect you will communicate in way that is professional and courteous and uses appropriate language. 

  2. Compliance 

    1. Everyone in the Online Community agrees to, and is expected to, read and act in accordance with our policies, including this Code of Conduct Policy and others found here and with State and National employment and health and safety laws. 

  3. Health & Safety

    1. Report unsafe environments, hazards and, should they occur, adverse events; 

    2. Take reasonable care in all activities you conduct and all professional interactions; 

    3. Promote and maintain a safe work environment and culture; 

    4. Uphold all obligations and responsibilities under the applicable health and safety laws, including precautionary measures to minimise hazards and risks; and

    5. Ensure you always act with care to maintain your wellbeing and the wellbeing of others.

  4. Media & Social Media

    1. We believe in the power of social media in promoting self-expression and advocacy but there are some things we will expect of you. You agree that:

      1. You will not make any public comment either on social or to the wider media where it could be construed that you are speaking on behalf of For Care or our Site. 

      2. You will not share any information via social media which will be a breach of confidentiality or privacy. 

  5. Privacy, Confidentiality & Information

    1. To allow For Care and For Care Providers to deliver the highest quality of support you agree to:

      1. Provide For Care and the For Care Provider will all required information relating to supports required, medication requirements, diagnosis, and more as needed. 

      2. Ensure all information provided is current and accurate, including, but not limited to:

        1. Assessments;

        2. Care Plans;

        3. Medical or other relevant referrals; and or

        4. Medical history. 

      3. Uphold the right to privacy of your For Care Provider by not taking advantage of your access to that information or disclosing their personal information to other parties.

  6. Professional Conduct

    1. At all times, you will:

      1. Be professional and ethical in your conduct with the For Care Online Community. 

      2. Act to protect the professional relationship between yourself, your family, your carers, friends and or delegates (including your Account Delegate is nominated) and your For Care Provider. 

      3. Not engage in any inappropriate behaviour, such as enter into any physical relationship, nor exploit the For Care Provider.

      4. Not act in any way which may be perceived as inappropriate or in which exposes your For Care Provider to any material which may be vulgar, graphic, sexually explicit, pornographic, culturally insensitive or otherwise offensive or inappropriate. 

      5. Report any incident or event  

  7. Respect

    1. At all times, you are expected and agree to:

      1. Respect your For Care Provider’s background and their personal preferences (such as their cultural or religious preferences). 

      2. Respect your For Care Provider’s right to privacy and freedom from inappropriate conduct. 

      3. Always act with integrity and treat all members of the For Care Online Community with respect and dignity.

ARE YOU A FOR CARE ACCOUNT DELEGATE?

As a member of our For Care Online Community you have been nominated as an Account Delegate for a For Care Customer. The For Care Customer has nominated you to assist in the registration and management of their online profile to connect with supports and services offered by For Care Providers. As such, this means you agree to:

  1. Accountability

    1. You agree to uphold the best interests of the For Care Customer that has appointed you their Delegate and only honestly represent and provide for their needs with their consent.

    2. You will not act in a manner that is contrary to the interests of For Care and the For Care Online Community. 

  2. Communication

    1. Ensuring all written and or verbal communication, whether via our Site or in the physical presence of others, is respectful, open and honest (whilst remaining in line with confidentiality and privacy. We expect you will communicate in way that is professional and courteous and uses appropriate language. 

  3. Compliance 

    1. Everyone in the Online Community agrees to, and is expected to, read and act in accordance with our policies, including this Code of Conduct Policy and others found here and with State and National employment and health and safety laws.

    1. You will not enter into transactions that may result a conflict of interest between yourself and the For Care Customer for whom you’ve been appointed. 

  6. Ethics

    1. We trust all members of our For Care Online Community to act truthfully and honestly whilst upholding the values of empathy, choice, control, independence and quality of life.

  7. Media & Social Media

    1. We believe in the power of social media in promoting self-expression and advocacy but there are some things we will expect of you. You agree that:

    3. Respect your For Care Customer & For Care Provider’s right to privacy and freedom from inappropriate conduct. 

    4. Always act with integrity and treat all members of the For Care Online Community with respect and dignity.

 

ARE YOU A FOR CARE PROVIDER?

As a member of our For Care Online Community you have registered to offer your services to For Care Customers using our Site. As such, this means you agree to: 

  1. Accountability

    1. You agree to only perform the tasks appropriate to your qualifications and experience and do not attempt work that exceeds your skills or knowledge. 

    2. You will not act in a manner that is contrary to the interests of For Care and the For Care Online Community. 

  2. Communication

    1. Ensuring all written and or verbal communication, whether via our Site or in the physical presence of others, is respectful, open and honest (whilst remaining in line with confidentiality and privacy. We expect you will communicate in way that is professional and courteous and uses appropriate language. 

  3. Compliance 

    1. Everyone in the Online Community agrees to, and is expected to, read and act in accordance with our policies, including this Code of Conduct Policy and others found here and with State, Territory and national employment and health and safety laws. 

  4. Conflicts

    1. You agree that gifts between yourself and your For Care Customer could be reasonably interpreted as inappropriate given the professional relationship and you undertake to not accept gifts and to ensure you notify your For Care Customer’s representative, carer, family Account Delegate, or other appropriate person(s). 

    2. You agree to avoid incidents where there may result in a monetary issue or conflict of interest. Particularly, you will not undertake responsibilities relating to the finances of your For Care Customer and will record information relating to any such potential or actual incidents. Particularly, should such an incident arise, you will notify your For Care Customer’s representative, carer, family Account Delegate, or other appropriate person(s).

  5. Ethics

    1. We trust all members of our For Care Online Community to act truthfully and honestly whilst upholding the values of empathy, choice, control, independence and quality of life.

    2. You will honestly record the hours you have worked in providing supports to you For Care Customers and understand your hours will be submitted to the For Care Customer for confirmation. 

    3. You agree to ensure your online profile is accurate and provides current information about your skills and experience.

  6. Health & Safety

    1. Report unsafe environments, hazards and, should they occur, adverse events; 

    2. Take reasonable care in all activities you conduct and all professional interactions; 

    3. Promote and maintain a safe work environment and culture; 

    4. Uphold all obligations and responsibilities under the applicable health and safety laws, including precautionary measures to minimise hazards and risks; and

    5. Ensure you always act with care to maintain your wellbeing and the wellbeing of others.

  7. Media & Social Media

      1. Provide For Care and the For Care Provider will all required information relating to education, qualifications, experience, availability and more. 

      2. Uphold the right to privacy of your For Care Customer by not taking advantage of your access to their information or disclosing that personal information to other parties.

  9. Professional Conduct

    1. At all times, you will:

      1. Be professional and ethical in your conduct with the For Care Online Community. 

      2. Act to protect the professional relationship between yourself, your family, your carers, friends and or delegates (including your Account Delegate is nominated) and your For Care Provider. 

      3. Not engage in any inappropriate behaviour, such as enter into any physical relationship, nor exploit the For Care Provider.

      4. Not act in any way which may be perceived as inappropriate or in which exposes your For Care Provider to any material which may be vulgar, graphic, sexually explicit, pornographic, culturally insensitive or otherwise offensive or inappropriate.

      1. Respect your For Care Customer’s background and their personal preferences (such as their cultural or religious preferences). 

      2. Respect your For Care Customer’s right to privacy and freedom from inappropriate conduct. 

      3. Always act with integrity and treat all members of the For Care Online Community with respect and dignity.
